## Job Description
The Social Worker is responsible and accountable for assessing the psychological, social, emotional, and/or financial needs that impact high-risk patient''s health and recovery. Addresses the patients assessed needs in a timely manner. Collaborates with the Case Manager in the planning of a discharge plan. Major Duties: Meets organizational expectations as defined by the Employee Standards of Performance, hospital and department policies and procedures, and the hospitals strategic goals. Verbalizes a clear understanding of continuous quality improvement and customer satisfaction measures and actively participates in improvement initiatives. Assessment of patient''s ability to cope with illness/hospitalization; evaluation of family support systems; provision of emotional support. Psychosocial assessment, including evaluation of family/home/financial support, and/or other high-risk social factor. Team member of the Weekly Interdisciplinary Meeting to aid in continuity of care after discharge to identify psychosocial needs. Provides crisis intervention; advocacy; counseling as needed. Knowledge of payor source impact on discharge needs and placement options including patient without healthcare coverage. Coordinates discussions/processes related to ethical or legal concerns (i.e. patient, family, team conflicts related to medical plan of care, guardianship cases, end-of-life decisions, advanced directives, and DNR orders). Coordination and patient support of abuse/neglect cases, domestic violence, substance abuse issues, crime victims, high-risk mothers, and adoptions. Referral to community support agencies/mental health services as indicated. Assists with placements to psychiatric facility, nursing home, SNF, rehab, LTAC, hospice, foster care, and shelters. Attends educational meetings to keep current on knowledge in field. Collaborates/assists the Case Manager with complex patient discharge needs. MSW practices in accordance with the LA State Board of Certified Social Workers'' rules and regulations upholding all SMH and Social Services policy and procedures. Responsible for getting Important Message from Medicare signature from patient within 48 hours of discharge. Performs other duties as assigned or directed to ensure smooth operation of the organization. Education/
Knowledge/Skills/Abilities: Preferred: Computer skills. Masters Degree in Social Work with LCSW with 1 to 3 years social work experience in an acute care setting. Required: Ability to assess information and apply critical thinking processes for course of action. Excellent interpersonal and written communication skills, able to prioritize workload and develop efficient methods of managing time. Ability to use phone, fax, and working knowledge of Outlook. Masters Degree in Social Work with GSW and 3 years general social work experience; 1 year experience in a healthcare setting. Licensure/Certification: Preferred: MSW, LCSW. Required: MSW, GSW.
