Social Worker (Grant & Per Diem)

Assigned to Mental Health Service whichdelivers care to Veteran patients on prevention, health promotion, coordinationand chronic disease management.
The Social Worker applies advanced professionaldiagnostic methods and evidence-based intervention strategies to a full rangeof patient problems.
Incumbent is responsible for the professional leadership andadministration of the Grant & Per Diem (GPD) component of the Health Carefor Homeless Veterans (HCHV) Program, which is administered by Mental Health Service(MHS).
The Grant & Per Diem Program provides on-going intensive casemanagement services that span the continuum from outreach and in-patientreferrals to residential treatment and permanent housing placement.
Theincumbent is also responsible for providing direct patient care to veteransenrolled in the GPD Program and their families.
He/she will manage veteransfrom a wide geographic area due to the transient nature of the homelesspopulation.
Incumbent oversees veterans with complex health care andpsychosocial needs requiring a high degree of clinical oversight and creativeproblem solving.
The incumbent is responsible for the day-to-day monitoring andcoordination of services provided by community-based programs funded under VA'sGrant and Per Diem Program and the National Homeless Program.
Monitorsprovisions for safe transitional housing and supportive services for homelessveterans.
Duties are performed in accordance withsocial work/psychological theories, techniques, and practices.
Theincumbent independently provides social evaluation/assessment and assistspatients and families in accessing VA and community resources by:
o Assessing veteran's social and emotional function and adjustmentto homelessnesso Identifying and planning for treatment of current and potentialareas of needo Recognizing age related needs (including cognitive, physical,emotional, and chronological maturational needs) of patients and adjustingtreatment accordinglyo Completing accurate and concise documentation on all patientrecords within Mental Health Service and facility guidelineso Facilitating optimal adjustment to issues such as grief andbereavement, chronic illness, permanent disability, chemical dependency or lifethreatening illnesso Identifying and addressing compliance issues; participating inhealth education, health promotion and wellness activitieso Maintaining an effective knowledge base of community resources andkeeping informed of changeso Managing each patients transition through the system andappropriately transferring accountability for follow up when patients aredischarged or transferred to another programo Developing a viable individual treatment plan in collaborationwith patients, families, interdisciplinary team members and community resourceso Assuming outreach responsibilities to community locations in whichassignedo Approving and insuring equitable service delivery among veteransreceiving specialized residential treatment for chronic mental illness andalcohol, drug dependency, or physical disabilitieso Insuring compliance with internal and external performancemeasureso Developing effective treatment strategies for veterans receivingresidential treatment through the HCHV/GPD program and their familieso Advising HCHV Program Director and Mental Health Director onrelevant programmatic issues with recommendations for improvement of servicesdeliveredo Collaborating with other program coordinators and managers withinthe Southeast Louisiana Veteran Healthcare Systemo Monitoring expenditures and obligations for the GPD Program o Placement of veterans into GPD community facilities includescontinued referral to vocational, substance abuse, medical and other servicesas neededWork Schedule:
7:
30am-4pm Monday-Friday , with weekend and evening tours.
Estimated Salary: $20 to $28 per hour based on qualifications.

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
